You are invited to participate in a research study about your physical and mental health during the COVID-19 pandemic. To be eligible for the study, you must be over the age of 18, have been diagnosed with any cancer, and be able to complete the study in English. The goal of this research is to understand the impact of the COVID-19 pandemic on your physical activity, behavioural patterns, and mental health. This study is conducted by researchers from the University of Toronto in the Faculty of Kinesiology and Physical Education.
This study will take approximately 45 minutes of your time. You will be asked to complete an online survey about the COVID-19 outbreak in your country/region and your physical and mental health. This will help researchers understand the impact of the COVID-19 pandemic on cancer survivors and identify resources to help survivors. You are free to skip any questions you do not feel comfortable answering or discontinue your participation at any point with no penalty, however any responses provided up to the point of withdrawal will be saved for analysis. All responses will be kept strictly confidential.

As a token of appreciation for your participation in this study, we are offering all study participants the opportunity to win a $25 gift card to Amazon. Six individuals who have participated in this study will be randomly selected to win 1 of 6 available gift cards for $25 CAD each. The odds of winning are dependent on the number of individuals who complete the study, but is predicted to be approximately 6 out of 300. This is optional. Any information provided throughout the study is strictly confidential and will be de-identified.
