FOR ALL FACE-TO-FACE (F2F) RESEARCH (ON OR OFF-CAMPUS):

This includes all in-person F2F research on or off-campus, including international research, where a UofT REB is the REB of record (not a TAHSN protocol). It does NOT include online or virtual research.

Complete the form, save the provided PDF and upload it as part of your MRHP protocol submission or amendment.

The completed form will be reviewed by the Face-to-Face COVID-19 Review Committee (F2FCRC) in a separate process from the REB's review of the human research ethics protocol. Approval of both the F2FCRC and the REB is required before F2F research can commence. If the study requires COVID-19 safety-related modifications, a member of the Committee may contact you directly. Otherwise, you will receive feedback in the Notes section of the MRHP application. Please note that the timeframe for approval of F2F research may be longer than normal due to the added F2FCRC review.

FOR OFF-CAMPUS (INCLUDING INTERNATIONAL) RESEARCH:

If your research project has an off-campus or international component but involves no face-to-face human research or human participants you should also complete this form.

Researchers are asked to send a copy of the completed form (provided as a PDF when the form is submitted) to ehs.office@utoronto.ca. Researchers will be contacted by OHS once that review has been conducted.

IMPORTANT REMINDERS FOR ALL SUBMISSIONS:

  • The form is used for both F2F and off-campus research but contains branching logic so that only relevant questions are asked.
  • For any research requiring international travel, consult with the Safety Abroad Office prior.
  • This process is not applicable for TAHSN researchers, TAHSN protocols or research conducted under the auspices of another institution (where that institution is the Board of Record).
  • This process is not required for virtual or online research.
  • This form is to be filled out by the Faculty supervisor or Faculty PI only (not to be filled out by students, trainees, research staff or post-docs).
  • Please note you can opt to save your draft as you go and return to it later by clicking "Save & Return Later" at the bottom of each page.
  • All Faculty supervisors/ Faculty PIs should keep a copy of their completed form for their records.
