INSTRUCTIONS:

Please prepare your appointment application as a single .pdf file named "Year_LastName_XApptApp", in the order below, and upload your document by November 15, 2020 in order to be included in the December Committee review. When uploading, ensure you click submit. If you have any questions, please contact projects.bme@utoronto.ca and/or refer to the Policies & Procedures for BME Cross-Appointments.

Documents to be included in your appointment application include:

  1. Completed appointment application form (a template can be downloaded here)

  2. Letter of intent OR summary of past contributions

  3. Signed letter of support from the head of your primary appointment (dated within the past 6 months)

  4. Signed letters of support from the head of each additional appointments you may hold (dated within the past 6 months; new applicants only)

  5. CV

  6. List of BME students supervised

Additionally, if you are a new applicant, please arrange to give a one-hour talk at the Graduate Speaker Seminars in order to introduce yourself to the BME faculty.
